Pulmonary Function Test
The Lung Health Check Pulmonary Function Test van is currently operational and accepting patients.
To book an appointment for your patient, please complete the requisition form below.
The requisition form can also be found in Accuro, Ava, CHR and PS Suite EMRs.
Once received, a representative from Aceso Medical will contact the patient with booking information.
Upcoming communities will be located in Northern Alberta and a schedule will be added soon.
